1) Create GotSport Roster for all players (including guest players).

Don’t forget to include Jersey # and Head Shots for each player. It is mandatory.
2) Upload PDF Official Roster (showing approval of governing body USYS, US Club)
3) Player Passes- Only needed for Guest Players. Please upload guest player passes in GotSport.
We will compare the gotsport roster you created and the pdf official roster you uploaded. If everything matches and all jersey number and headshots are present, then we will approve each player. This approved roster is what will appear on the game cards for the officials each game. If any information is missing, we will send you a follow up message through GotSport. Please be checking your emails throughout the process.
Reminder: Rosters are due Monday, May 12th.
GUEST PLAYERS
On PDF official roster, hand write player name, jersey number, DOB, and ID #.

Teams may roster up to 22/18/14 players for their specific tournament division (see Rules). If there are players on your roster not attending the event, please cross those players off the uploaded official roster.
Up to 6 guest/club pass players are allowed per team, as long as the total number including those players does not exceed the total allowed number of players.
TRAVEL PAPERS
Permission to travel (if coming from outside Region IV). Please contact your state association for proper form and procedure.
Player loan forms (if required by home State association). Please obtain from home state association.
Interstate Permission to Travel Form (for a player to play with another club outside of their home state. Example, New Mexico player playing with a Texas team).
US Club did add a Player Loan Form to their requirements regarding CLUB-to-CLUB US Club guest players. It is required by US Club to be completed and submitted to ensure US Club Insurance Coverage for the guest player. Please click on the Player Loan Form to be fully informed.
Medical Release Form
